Neet Application and paymentSpecifications of the photograph:

Important note:
Same Passport size and Post Card size photographs are to be used for uploading with Online Application Form and for pasting on Attendance Sheet at the Examination Centre/Counselling /Admission. Candidates are reminded to keep 6 to 8 passport size and 4 to 6 Post Card size (4”X 6”) color photographs with white background.
  1. Date of photograph taken should be mentioned in the photo (After 01.09.2019 photograph should be taken)
  2. The focus needs to be on face (80% face coverage, ears clearly visible, on white background)
  3. The photographs need not be attested
  4. Spectacles are allowed when it is used regularly
  5. Not allowed:
    • Photograph with cap or goggles
    • Polaroid and Computer-generated photo
  6. The application will be rejected in case of:
    • Unclear photographs or not complying with the above-mentioned instructions
    • Application without photographs
    • Photographs uploaded are found to be fabricated
  7. Candidature shall be canceled and the case of someone else’s photographs have been uploaded (Criminal action as per laws of the country shall also be initiated)
Specifications of the Signature:
Full signature in running handwriting (not in capital letter) on white paper with Black Ink pen and scan for uploading. Online Application Forms will be rejected when unsigned Candidature shall be canceled: In case someone else’s signature has been uploaded (Criminal action as per laws of the country shall also be initiated)
Specifications of the Thumb Impression:
His/her left-hand thumb Impression on white paper with Blue Ink and scan for uploading.

Ensure that the uploaded images are clear and proper right-hand thumb impression may be used, In case of any eventuality of left thumb being unavailable.

Online Application Forms will be rejected when unclear Left-hand thumb Impression Uploaded

Instructions for completing the Online NEET-UG Application Form:

  • Mode: Application Form other than the one filled in online mode will not be accepted.
  • Confirmation Pages: candidates are advised to preserve at least four copies of the Confirmation Page along with the proof of fee paid for future reference.
  • Eligibility: The candidate must ensure his eligibility to appear in the test before submitting the application form online (Check the bulletin carefully).
  • Information to be filled in the Online NEEt-UG Application Form:
    1. Candidate’s Name (with correct spellings as given in the Secondary School Examination or equivalent Board/ University Certificate in capital letters).
    2. Names of Father/Mother and Candidate (with correct spellings as given in the Secondary School Examination or equivalent Board/ University Certificate in capital letters).
    3. Date of Birth (dd/mm/yyyy).
    4. Valid Identity Card: Aadhaar Number (last 4 digits)/Class XII Roll number issued by the Board/Election Card (EPIC No.)/Passport number/Ration Card Number/ Bank Account Number/number of Other Photo Identity Card issued by Government.
    5. Permanent address with the pin code
    6. Mobile Number (own or parents only for future correspondence)
    7. Email id (own or parents only for future correspondence)
    8. NTA will communicate through SMS or e-mail on the given mobile number and e-mail ID.
    9. Candidate should not give the postal address, contact number/mobile number, and e-mail ID of Coaching Centres in their Online Application Form.
  • Self Declaration: Candidates from UT of J&K and Ladakh are required to submit the online Self Declaration.
  • Qualifying Examination:
    1. Candidates should fill appropriate Code 02 to 07 according to the Qualifying Examination you passed.
    2. Candidates appearing in Class XII in 2021 should select qualifying code as 01.
    3. NB: candidate needs to furnish complete details under the column ‘others’ when does not fall within the qualifications prescribed as per Code numbers 02 to 07.

  • A candidate who is appearing in the qualifying examination, i.e., 12th Standard in 2021, whose result is awaited, may apply and appear in the test but he/she shall not be eligible for admission to the Undergraduate Medical Courses if he/she does not pass the qualifying examination with the required pass percentage at the time of first round of Counselling.
  • Candidates who have appeared or are appearing at the qualifying Examination with English, Physics, Chemistry, and Biology as main subject and expect to pass the Examination with required percentage of marks are also eligible to apply and appear in the Competitive Entrance Examination.  However, their candidature will be considered only if they provide documentary evidence of having passed the qualifying Examination with the required subjects and percentage of marks.
Code: 02 
  • The Higher/Senior Secondary Examination or the Indian School Certificate Examination which is equivalent to 10+2 Higher / Senior Secondary Examination after a period of 12 years study, the last two years of such study comprising of Physics, Chemistry, Biology / Bio-technology (which shall include practical tests in these subjects) and Mathematics or any other elective subject with English at a level not less than the core course for English as prescribed by the National Council of Educational Research and Training after introduction of the 10+2+3 educational structure as recommended by the National Committee on Education.
  • Candidates who have passed 10+2 from Open School or as private candidates shall not be eligible to appear for the ‘National Eligibility Cum Entrance Test’. Furthermore, the study of Biology / Biotechnology as an Additional Subject at 10+2 level also shall not be permissible.
  • If the institute has to consider an examination of an Indian University or of a foreign / university to be equivalent to the 12th Class under 10+2 scheme/intermediate science examination, the candidates shall have to produce a certificate from the concerned Indian University / Association of Indian Universities to the effect that the examination passed by him/her is considered to be equivalent to the 12th class under 10+2 scheme/intermediate science examination.
Code: 03
  • The Intermediate / Pre-degree Examination in Science of an Indian University / Board or other recognized examining body with Physics, Chemistry, Biology /Biotechnology (which shall include practical test in these subjects) and also English as a compulsory subject.
Code: 04
  • The Pre-professional / Pre-medical Examination with Physics, Chemistry, Biology / Biotechnology and English after passing either the Higher Secondary Examination or the Pre-University or an equivalent examination. The Preprofessional/Pre-medical examination shall include practical tests in these subjects and also English as a compulsory subject.

  • The first year of the three years’ degree course of a recognized University with Physics, Chemistry and Biology  /Biotechnology including practical tests in these subjects provided the examination is a University Examination and candidate has passed the earlier qualifying examination with Physics, Chemistry, Biology/ Biotechnology with English at a level not less than a core course.

  • B.Sc. Examination of an Indian University provided that he/she has passed the B.Sc. Examination with not less than two of the subjects Physics, Chemistry, Biology (Botany, Zoology) / Biotechnology and further that he/she has passed the earlier qualifying examination with Physics, Chemistry, Biology, and English.
Code: 07
  • Any other examination which in scope and standard (Last 02 years of 10+2 Study comprising of Physics, Chemistry and Biology / Biotechnology; which shall include practical test in these subjects) is found to be equivalent to the Intermediate Science Examination of an Indian University / Board, taking Physics, Chemistry and Biology/Biotechnology including practical tests in each of these subjects and English.

Important Notice:

  • No changes allowed: Request for change in any particulars in the online application shall not be entertained under any circumstances.
  • Rejection/cancellation: Incomplete Applications shall be rejected or submission of more than one application form by an individual candidate will be canceled
  • Suspicion: any doubt arises that a candidate has filled in multiple forms or impersonated or not provided genuine information or given fake or false, information or the likes such a candidate can be asked to give additional proof of his identity at any point of time.
  • Duplication/impersonation/ fabrication of facts: duplicate forms or has been involved in any form of impersonation or fabricated facts, such candidate shall be liable for severe penalty including being debarred from taking the NTA examination for a period of three years/Such candidates shall also be liable for suitable criminal action under relevant laws of the country.

Common services centers/facilitation centers:

  • Services of Common Services Centre, Ministry of I.T., Government of India under the Digital India initiatives may be utilized in areas, where candidates find it difficult to submit online applications due to various constraints.
  • Common Services Centres will provide (on a small fee) the desired support to candidates in online submission of application form and payment of fee through e-wallet.
  • The list of the Common Services Centre is available on the website: csc.gov.in.
